PracSuite Clinical Notes allow you to add a configurable Matrix component. This allows you to ask a series of related questions and calculate a score based on the answers.

Begin by adding the component to your Clinical Note Template via the Add To Note button at the top.

When you add the component, some defaults appear to help you understand how it will display. Start by giving it a Label.

The switch for Assigning number values for choices is on by default, this will allow calculations, switch it off if it is not required.

PracSuite Matricies can handle multiple sections, which allows you to have an advanced calculation across different question and choice combinations. Use the add button above Section 1 to add an additional set of questions and choices.

In each section, you start with three Choices, edit these and use the Add button to create more, or use the trash can icon to remove them.

Next, configure the Questions, as with the choices, you begin with three. Edit the questions and use the Add button to create more, or use the trash can icon to remove them.

Finally, you can Include totals on results. Turn this off if it is not required. By default you have the grand total and column totals programmed for you, if you do not want to see these untick the checkboxes in the display column. You can also add in your own calculations, simply select Add, give it a title and then write in a custom calculation referencing previous answers to your questions above using their reference letter (A,B,C in the above screenshot).

Once you are done, select Save and preview it on a test patient before using it.
