Using a Calculator in Patient Forms

Forms allow you to add a configurable Calculator component. Some examples of using this component include defining NDIS budgets on service agreements, constructing a table of rates and calculating a client's Body Mass Index (BMI).

Included at the bottom of this article are samples of an NDIS budget intended for service agreements and a BMI calculator. Note that you will need to customise and edit the values for your profession)

Begin by adding the component to your Form Template using the Add To Form button at the top.

How to configure a calculator component on a patient form

Label your calculator and use the switch to decide if a patient should be able to see the calculation fields.


Configuring Columns

The first step is to add a Column using the ADD button.

Use the type dropdown to decide what data is presented in this column. The first column is named A, and subsequent columns you add are given a new letter alphabetically. Once you have selected the Type of column, you configure a column using the cog button or select the Configured per row option if you want to use custom settings on each row.

The different Column data types are:

Label

Use the Label option for guiding someone through your calculator. You can leave the column header label blank and customise the entry on each row.

Number

Define the minimum and maximum, how many decimal places are required, and add a prefix or a suffix.

Drop Down

The drop-down option is great for having a text-based selection tied to a value.

Calculation

The engine of the Calculator. Use + - * and / to create a formula for your calculation element, and then use totals at the bottom of the template to perform complex calculations.


Configuring Rows

Once you have added at least one column, you can add a Row below your column list.

Each row is given a number that you can use to reference a specific cell in a calculation, e.g., A1, B1, C1. You can prefill the form here by editing individual values in any label, number or dropdown row. If you have selected Configured per row on any columns, you will see a cog next to each section you can configure.


Allow Patient to Edit

Unique to forms, the calculator component allows you to specify whether a patient can edit a field. After selecting the cog on a column (unless configured by row), you will find this option as a checkbox.

Disallowing patients to edit sections allows you to use the Edit Before Sending option when manually sending patients' forms. This is ideal for NDIS service agreements as you can fill it in and then send the patient a read-only version for them to sign.


Totals

In the Totals section, you will find a preset calculation for a Grand Total (every value figure added together) and column totals. If you don't want these to display, untick the Display box.

Use the Add button to create your own calculations. Use the Display to Patient option to choose whether the calculation is shown to patients completing the form, or only to staff within PracSuite.
